Job Details: The Registered Nurse provides and coordinates acute skilled nursing care for patients of all ages in their homes utilizing nursing principles that recognize the psychosocial, economic, cultural and individualized health status and family/support systems and resources that affect health status and results in maximum outcome achievement. The RN serves in a supervisory capacity by directing and evaluating paraprofessional staff. The RN plans, provides, evaluates, coordinates care and provides case management and documents in a manner which meets the quality, efficiency, and care standards identified by Kno-Wal-Lin, as outlined in Agency Policies and Procedures and according to professional standards of home health nursing practice. The RN acts as case manager and is responsible for overseeing the ongoing care planning management, coordination of care, communications and for outcomes of the plan of care. The RN acts to uphold the Agency’s mission, standards of care and fiscal soundness within a changing health care environment.
Minimum Qualifications: Graduate from an NLN accredited nursing program. One year of clinical experience required. Additional experience in nursing case management, home care, hospice, and acute care areas desirable. CPR certification or eligibility; IV certification or eligibility. Current license to practice as an RN in the state of Maine. Ability to work independently, with minimal supervision, in home settings presenting diverse physical conditions and socio-economic environments. Work with patients/families of all ages and cultures. Current Maine drivers license, reliable transportation and proof of current agency acceptable motor vehicle liability insurance required Regular travel involved in daily activities.
